The purpose of this grant was to develop an effective health information technology (health IT) system that can be replicated and serve as a model of community cooperation. The entities in the Health Improvement Collaboration consisted of health-related and health safety net member organizations located in north and eastern Oklahoma. The scope of the project was: (1) Develop an integrated health care system across traditional boundaries; (2) Enable the community to provide health services across boundaries; (3) Improve the quality of health care and increase patient safety; (4) Identify key health issues through a data base integration system; (5) Describe population health in detail and with current data, leading to targeted strategies aimed at improving health outcomes; and (6) Increase funding opportunities through infrastructure development. The network worked toward problem decomposition by developing a system of independent sub-goals to determine work needed to combine results into sub-plans. The network had to determine observable, deterministic, finite, static, and discrete environments of the project to keep within relevant ongoing actions. Heuristic functions were observed and reviewed effects of planned actions/ goals and made educated "guesses" to determine actions needed to resolve or achieve goals. Members participated in formal needs assessments to identify a responsive health IT system. Members facilitated meetings with outside agents/ agencies to access "Best Practices," HIPAA Compliance, system development and general resourcing for achievement. A needs assessment was conducted relative to available resources and skills. A plan was formalized for the Community Health Information Network, and a business and strategic plan were put in place. In addition, clinical system integration issues were identified and addressed; interagency and community-based governance models were developed; and barriers related to shared information systems were addressed.
