This project developed a tool to promote activation, communication, engagement, and self-management of pediatric blood and marrow transplant patients and their parents and found that patient-centric tools can successfully engage caregivers in hospital care.
Established a Web-based electronic medical record system for 10 small rural hospitals to connect them to the area's regional medical center. The project's ultimate goal was to quickly give all providers access to patient data, eliminate duplicate tests and exams, deliver high-quality care, reduce medical errors and track health outcomes.
Develops a patient Master Visit Registry (MVR), addressing the need for better information sharing among clinical organizations and enhancing their ability to give patients continuous high-quality care when they change providers. The MVR will expand upon an existing patient record-keeping system, while improving local handling and exchange of records.
Developed approaches to share data on patient clinical and diagnostic information across systems and created an implementation plan for systems integration.
Examined the use of the HANDS software system, a health IT-supported care planning process for nursing care, and its ability to be transferable between nurses, units, and health care settings.
