This project will formulate evidence-based recommendations for clinical decision support used by community pharmacist delivering medication therapy management. The goal is to reduce medication-related problems and improve health outcomes for chronically ill patients.
This study evaluated the effectiveness of an electronic medication reconciliation intervention by comparing outcomes pre- and post-implementation in six community-based primary care clinics and two inpatient facilities.
The project developed and pilot-tested a Web-based implementation of a Team Resource Management (TRM) intervention aimed at improving medication safety in primary care.
Created a secure infrastructure for communication among providers to allow electronic sharing of patient clinical information with hospitals and other physicians/health providers in the county, region, and State; also assessed the effectiveness of the system in improving workflow, timeliness and completeness of information, patient safety, continuity of care, and health outcomes.
Implemented the newly developed Electronic Vision Rehabilitation Record and its tools to evaluate the effectiveness of current best practices and help refine practice as the evidence indicates.
