Asthma, Overdue for Action Plan Review

Clinician Summary

AAP practice guidelines recommend regular follow-up for children diagnosed with Asthma. These visits should occur at least every 3-6 months depending on symptom severity, at which time physicians should reevaluate symptom severity, and appropriate medication types and doses. Many patients do not have regular follow-up to reassess symptom severity and the need to adjust medications. The goal of the reminder is to alert a physician that a patient has not had a visit to reevaluate symptoms and medications in at least 6 months. The reminder can then be extinguished for a predefined period of time based on a physician’s actions

This reminder prompts clinicians to review the action plan for individuals who have the following recorded indicators within the electronic health record (EHR) system:

  • age < 19 yrs; and
  • asthma is on problem list or medication list includes albuterol or asthma medication; and
  • last documented visit for asthma action plan review > 12 months ago
