Description:

Service Integration

Description: 

Built an integrated communications system with area hospitals, clinics, and service providers in St. Mary Parish, Iberia Parish, and Terrebonne Parish that will support chronic disease management and improve patient safety. State and Regional Demonstration in Health Information Technology: Indiana

Description: 

The Indiana Network for Patient Care, an operational health information exchange (HIE) in central Indiana, is one of six AHRQ sponsored State and Regional demonstration projects begun in late 2004 and early 2005 to create State or regional HIEs.

Description:

EMR Planning to Improve North Iowa Health Care

Description: 

Designed a system-wide patient-centered planning process and an EHR implementation plan that securely exchanged patient information within and across diverse health care settings for the Hancock County Memorial Hospital, in addition to 21 affiliated physician health organization clinics.

Use of Electronic Referral System to Improve the Outpatient Primary Care-Specialty Care Interface

Description:

Implementation of Health Improvement Collaboration in Cherokee County, OK

Description: 

Implemented a HIPAA-compliant electronic records system to improve public health, patient safety and facilitate information sharing among health care providers in the Talequah area, with the goals of establishing a Web-based information and referral service and creating a community-wide, science-based prevention strategy supported by the new information system.
