A National Web Conference on the Innovative Use of Technology To Enable Behavioral Changes in Hard-to-Reach Patients

Overall Purpose:

Through multiple mechanisms, including the judgment of the continuing education (CE) planning group, the Agency for Healthcare Research and Quality (AHRQ) has identified a gap in a health care/public health practitioner's competency within the health IT environment. This series of Webinars is designed to increase practitioners' competencies to improve health care decisionmaking, support patient-centered care, and improve the quality and safety of medication management. This session increases knowledge about the methodologies, achievements, and lessons learned of a series of projects that employed communication-focused technologies to engage hard-to-reach populations. Specifically, the projects presented will discuss the adaptation of a clinical patient education system to deliver a behavior change intervention for young African-American women, investigating the use of telehealth kiosks to aid older adults in managing hypertension and other chronic conditions, and the implementation of a short message service (SMS)-based intervention which provided tailored health-communication messages for HIV-positive men who have sex with men (MSM).

Learning Objectives:

  1. Describe challenges associated with employing Virtual support programs within a specific patient population.
  2. Discuss the importance of using SMS communications to engage ambulatory patients in hard-to-reach populations.
  3. Describe the improved outcomes that may be seen within a particular patient population by deploying telehealth technologies to increase adherence and involvement in care.
  4. Identify lessons learned and ways in which the findings and achievements of these projects may be utilized in your local environment or similar projects that you are pursuing.
