Tools and Technology for Improving Care Post-Treatment (Technical Assistance Event)

Overall Purpose:

Coordination of care post-treatment is essential to patient safety and is increasingly the focus of national quality measurement and requirements. Whether it be providing patients with adequate information regarding post-treatment follow up and medications, helping patients adhere to care plans, or ensuring that the care team has all the required information on a patient, the area of post-treatment care provides opportunities for improvement and for health care services research.  Health information technology (IT) can play a key role in streamlining and improving post-treatment care through various tools and technologies such as patient portals, online patient education, electronic discharge summaries, and patient reminders. To meet the needs of its grantees in the area of care coordination and continuing care, the AHRQ Health IT Portfolio sponsored a Webinar to provide an overview of the current health IT landscape as it pertains to care transitions and health services research as well as to present some specific health IT tools and technologies used for improving care post treatment.
