This project evaluated the readiness and feasibility of EHR data for ambulatory clinical performance measurement in a community-wide, multi-payer HIE.
This project sought to identify existing, and create new, clinical quality of care metrics able to be reliably retrieved from electronic systems, such as electronic health records and health information exchanges.
This project demonstrated the ability of an interoperable health information exchange and an electronic health record to provide useful quality and safety measures for the vulnerable populations served by two community health center clinics.
This project sought to develop an automated self-management monitor and test its impact of on glycemic control and self-management behaviors among patients with type 2 diabetes mellitus.
The findings of this study demonstrated that electronic health record-based trigger methods can enable more meaningful measurement and surveillance of diagnostic errors in primary care.
This project designed and validated a new diabetes quality of care process measure.
This study supported the notion that the current focus on quality of care for diabetics favors providers that start with panels where a majority of patients already have good control.
Citizens Memorial Healthcare (CMH) will establish the standardization efforts necessary for data capture of quality measures in an ambulatory EMR system and demonstrate the efficiency and accuracy of using a data extraction and reporting expert to perform quality measurement in the ambulatory care setting. CMH will standardize and integrate data capture for quality of care measurement into the normal documentation of care within the ambulatory EMR.
This project refined a set of asthma care quality measures and developed and validated the use of an automated method using natural language processing to utilize the measures.
