Using IT to Improve the Quality of Cardiovascular Disease (CVD) Prevention and Management

Description: 

This study developed electronic medical record-based quality indices for eleven cardiovascular primary care services. It related physicians’ prior index scores to subsequent disease incidence and to care utilization in their patients.

Impact of Health Information Technology on Clinical Care

Description: 

Evaluated the effects of staggered installation of an Epic health IT system that includes an EMR with provider order entry and clinical decision support in primary care settings on quality, safety, and resource use within a large integrated delivery system on cohort of 780,000 members with chronic illnesses.

Improving Quality in Cancer Screening: The Excellence Report for Colonoscopy

Description: 

This project was designed to evaluate and improve the quality of screening and diagnostic colonoscopies in ambulatory care settings. Defined quality measures for colonoscopy were used in individual quality report cards with 15 measures created.

Medication Monitoring for Vulnerable Populations via Information Technology (MMITI)

Description: 

This project demonstrated the ability of an interoperable health information exchange and an electronic health record to provide useful quality and safety measures for the vulnerable populations served by two community health center clinics.

Bringing Measurement to the Point of Care

Description: 

This project deployed new functionality that allowed clinicians to view ‘real time’ measures of the delivery of clinical preventive services and added patient specific alerts and ‘one click’ options to act on missed opportunities for followup care.
