Health IT Survey Compendium
The Health IT Survey Compendium provides a centralized resource of publically available health IT surveys, many of which were developed by AHRQ-funded projects. Surveys may be used as is, serve as templates to create new surveys, or questions pulled out and used on their own.

Description: The purpose of this project was to examine the feasibility of developing an electronic health information network to provide clinicians with patients' medical information during care transfers and found that, while feasible, there are significant barriers to information exchange.

Description: Facilitates transfer of information among providers and patients in the Presque Isle community; implements a model of chronic care management; and educates area health care providers on how best to use current information systems to communicate with each other.
