Health IT Survey Compendium
The Health IT Survey Compendium provides a centralized resource of publically available health IT surveys, many of which were developed by AHRQ-funded projects. Surveys may be used as is, serve as templates to create new surveys, or questions pulled out and used on their own.

Description: Implemented an interactive video-conferencing system at rural hospitals to provide continuing education for pharmacist and pharmacy technicians as well as a model for bedside verification of medication administration and medication bar coding; also evaluated structure, process, and outcomes related to improvement of patient safety and more effective patient medication management.

Description: Shared an electronic medical records system that improved patient safety and quality of care. Also served as a critical learning tool for clinicians in a coalition of three large health organizations and 24 primary care clinics in northern Iowa.

Description: Implemented a series of new health information technologies in carefully staged processes over 2 years to include an Inpatient Pharmacy System, Electronic Medication Administration Record, Bar Coding System, and a CPOE System; evaluated the impact of these systems on safety, quality, and efficiency.

Description: Planned the development and implementation of a health IT infrastructure throughout three rural counties, including high-speed Internet access, CPOE, CDSS, EHR, and continuity of care record templates.

Description: Built upon an existing infrastructure to construct a fully integrated EMR to give clinicians real-time access to patient data through pharmacy management, laboratory management, patient scheduling, barcoding, clinical physician order entry, electronic signature, insurance eligibility, and Pyxis medication-dispensing units at nursing stations.

Description: This research created, piloted, and evaluated FIQS, the Family Input to Quality and Safety tool, that allows pediatric patients and their caregivers to provide safety reports regarding their inpatient care.

Description: This project developed a tool to promote activation, communication, engagement, and self-management of pediatric blood and marrow transplant patients and their parents and found that patient-centric tools can successfully engage caregivers in hospital care.

Description: This study assessed the usability and impact of inpatient portals on patient experience, engagement, and perceptions of care.
