The purpose of this project was to examine the feasibility of developing an electronic health information network to provide clinicians with patients' medical information during care transfers and found that, while feasible, there are significant barriers to information exchange.

This case study examined four different HIE organizations to explore stakeholders’ perspectives on benefits and barriers of participating in health information exchange.
In this project, the National Quality Forum reconvened the Health Information Technology Expert Panel (HITEP-II) to gather, synthesize, and refine clinical workflow maps, focusing on care processes related to the previously prioritized set of measures.
Developed a community-wide HIE collaborative in a rural area that gave patients and providers access to comprehensive clinical data on the Internet; developed disease-management prototypes on diabetes, pediatric asthma, depression, and low back pain and evaluated the development, implementation, and outcomes of the collaborative.
Created a detailed assessment of the feasibility of health IT implementation including the development of an implementation plan, specification of clinical and organizational needs, identification of goals, and identification of barriers and ways to address those barriers.

Through the Patient-Centered Medical Home (PCMH) Information Model project, Westat aims to develop a functional model to define the PCMH that would describe it in both narrative and technical terms through the interactions between and among PCMH stakeholders.
This project extensively tested, refined, and evaluated a tool called the Hazard Manager, a tool designed to support the characterization of hazards and communicate their potential and actual causality in adverse effects.
