With the advancement of digital technologies, patients can easily collect their own health data outside of the clinical setting to track and manage their health. While these technologies are promising, many ambulatory care practices lack the technical infrastructure, functional workflows, workforce capacity, and training to support the intake and use of patient-generated health data (PGHD).

Improving the Management of Multiple Chronic Conditions with mPROVE

Description

This project developed and piloted Mobile Patient-Reported Outcomes for Value and Effectiveness, a bilingual mobile patient-reported outcomes system integrated into primary care for ethnically and culturally diverse adult patients with multiple chronic conditions. It proved feasible, acceptable, and effective in enhancing communication and decision-making, while revealing usability and implementation challenges to address for broader adoption.
