The Computer Assisted Medication and Patient Information Interface project developed and tested “My Medication Helper”, an innovative computer assisted self-interview tool delivered via a kiosk for use in a hospital diabetes clinic.
Developed a community-wide HIE collaborative in a rural area that gave patients and providers access to comprehensive clinical data on the Internet; developed disease-management prototypes on diabetes, pediatric asthma, depression, and low back pain and evaluated the development, implementation, and outcomes of the collaborative.
Safety Through Enhanced e-PreScribing Tools (STEPStools): Developing Web Services for Safe Pediatric Dosing
The project team successfully developed a knowledgebase specific to medication management for children. The tools created included an algorithm for rounding of medication doses and a compounded medication knowledgebase.
Implementing Personalized Cross-Sector Transitional Care Management to Promote Care Continuity, Reduce Low-Value Utilization, and Reduce the Burden of Treatment for High-Need, High-Cost Patients
This research will integrate cross-sector care alerts and interoperable personalized care planning into the existing Coordinating Transitions Intervention (CTI) tool and evaluate the impact of the revised tool on patient burden, care team collaboration, and utilization value for high-need, high-cost patients.
This research will develop and evaluate a machine learning-augmented and telemedicine-augmented sociotechnical intervention for postoperative handoffs to reduce the risks of patient complications and improve patient-centered care.
